Medical Billing KPIs & Benchmarks Every Practice Should Track in 2026

Your revenue cycle generates a lot of numbers, but only a handful actually predict whether your practice gets paid fully and on time. Track these KPIs, compare them to industry benchmarks, and you’ll spot revenue leaks long before they reach your bank account. (Benchmarks vary by specialty and payer mix — treat them as targets to trend toward, not absolutes.)

Claim quality & first-pass metrics

  • Clean Claim Rate (CCR). The share of claims accepted on first submission with no edits. Benchmark: aim for 95%+ (top performers exceed 98%). It’s the best leading indicator of how fast you’ll be paid.
  • First-Pass Resolution Rate (FPRR). The share of claims paid the first time, without rework or appeal. Benchmark: 90%+ is solid, 95%+ is excellent. A low FPRR means expensive manual rework.

Denial metrics

  • Denial Rate. The percentage of claims denied by payers. Benchmark: keep it under 5–10%; best-in-class practices stay below 5%. Rising denials are the clearest sign of front-end or coding problems — see our denial codes cheat sheet for the most common culprits.
  • Denial Overturn Rate. The share of appealed denials you successfully recover. Benchmark: a large portion of denials are appealable — if you’re not working them, you’re writing off recoverable revenue.

Accounts receivable

  • Days in A/R. The average number of days to collect a dollar after the date of service. Benchmark: under 40 days is healthy, under 35 is good, top performers run under 30. Rising days in A/R signals slow billing or follow-up.
  • A/R over 90 days. The percentage of total receivables aged more than 90 days. Benchmark: keep it under 15–20% of total A/R. The older a balance gets, the less likely it is to ever be paid.

Collections & cost

  • Net Collection Rate (NCR). Of the money you were contractually allowed to collect, the percentage you actually collected after legitimate write-offs. Benchmark: 95%+ is standard, 97%+ is excellent. This is the truest measure of billing effectiveness — a low NCR means earned revenue is slipping away.
  • Cost to Collect. Your total billing cost as a percentage of net revenue collected. Benchmark: roughly 3–5% is typical; the goal is to lower it without hurting your collection rate.
  • Point-of-Service Collection Rate. The share of patient responsibility collected at or before the visit. Benchmark: with high-deductible plans now common, collecting up front is far cheaper than chasing balances later.

How to turn KPIs into action

Numbers only help if you act on them. The practices that stay financially healthy do three things:

  • Review the same dashboard every week — not once a quarter, when a problem is already entrenched.
  • Trend, don’t just snapshot — a single month tells you little; the direction over time tells you everything.
  • Give every KPI an owner and an action so a bad number triggers a fix, not just a frown.
